Event Details

Representatives from the Family Health Partnership Clinic will be at the library 1-4pm on Thursday, October 7 to provide information about the McHenry County COVID-19 Community Response Project. 

COVID-19 has disproportionally impacted vulnerable individuals and communities, COVID-19 Community Response Project helps to ease the navigation of care resource coordination. Care resource coordination is an integral part of connecting the most vulnerable populations impacted by the COVID-19 pandemic and helping to improve outcomes. The FHPC representatives will be in the library to explain available community resources.
